BILLING AND INSURANCE INFORMATION
Our policy is to ask that fees for office visits be paid at the time of service. Although charged separately, fees for additional services such as x-rays, casting and durable goods are also payable at the time of service. Please keep in mind that cash, personal checks, money orders, Visa and MasterCard are accepted. Any insurance benefits you may receive are strictly between you and your insurance company. In special cases such as workers compensation and injuries under litigation, please inform the secretary before seeing the physician.
